What is SMS Confidential Reporting

As an employee who works at AMA, we all have a due diligence to protect every person who steps on our airport. It is imperative to identify and assess the level of risk associated with potential hazards. This can be done quickly and confidentially by filling out and submitting an SMS Reporting Form.

Disclaimer

All persons submitted or listed on the form will remain confidential, unless it is not legally permissible to do so. In the event of an emergency, always contact 911. If the risk or hazard is not an immediate emergency, complete the SMS Reporting Form.

Why it is important

SMS Confidential Reports provide detailed descriptions of events that need to be brought to the attention of the Airport’s SMS staff, so that they can be assessed for overall risk. Taking this proactive approach enables the airport to identify, analyze, and address each risk minimizing it or its potential before it should occur.

Directions for filling out the report

1. Access the SMS Reporting Form here.
2. Pull the drop down menus for all of the required information necessary in the report and be as detailed and descriptive as possible.
3. If you desire you may include information in the optional fields to complete your report.
